Hospice Care
Pine Meadows Post Acute has a hospice team that can come into the facility when requested to treats residents suffering from serious and chronic illnesses. This hospice team focuses on helping each resident gain the strength needed to carry on with daily life. They strive to facilitate relief from the symptoms and stress of the serious illnesses our residents suffer with.
Hospice care uses a team approach to care. This team may include:
- Doctors
- Nurses
- Social workers
- Therapists
- Pharmacists
- Nutritionists
- Chaplains and others as needed or requested.
This team spends as much time as necessary with each resident and their family. Our facility will and hospice team will support each individual in our care every step of the way, not only by controlling symptoms but also by helping with treatment options and goals.
